An important element that affects the healing process and the patient’s subjective perception of the treatment process is the appropriate selection of a specialized dressing, which takes into account its structure. The paper described selected forms of dressings (creams, ointments, hydrogels) and the possibilities of their application in practice. Methods of application and the possibility of combining them with other dressings were considered. Elements of the TIME strategy, which promotes the support of natural healing mechanisms, should be used to systematize the management of hard-to-heal wounds and the choice of dressing at different stages of healing. By combining systematicity in handling each dressing change and its correct application, optimal conditions for wound healing can be attained. key words: speciality dressings, silver, chronic wounds |

An intestinal stoma is a deliberately performed connection of the intestinal lumen with the surface of the body as a result of a surgical procedure. The proper functioning of a stoma has a large impact on its acceptance by the patient and on his/her quality of life. Despite the continuous improvement of surgical techniques and appropriate preoperative preparation of the patient, complications of the skin around the stoma are still the biggest problem for the patient. They occur with high frequency and result in leakage of ostomy equipment and increased cost of stoma care. Complications of the stoma in many cases can be effectively prevented. Wound dressing is a field of medicine that is often underestimated by modern researchers. Due to the use of a sterile operating field, antiseptics and antibiotic therapy, the fear of infections – common hundreds of years ago – has receded into the background. The principles that allow modern surgeons such comfort have been refined over the centuries by medical pioneers. One of them was one of the most famous doctors: Jan Mikulicz-Radecki, whose nationality is disputed till today. Besides merits such as the construction of a gastroscope, chambers for thoracic surgery or describing surgery techniques, he was also famous for techniques of antiseptic wound dressing. This article describes the methods used in the treatment of wounds in the Surgical Clinic in Cracow during the management by Jan Mikulicz-Radecki. It focuses on antiseptics such as: carbol, sublimate and the most commonly used iodoform. Wound treatment should be based on the TIME strategy including proper wound assessment and debridement, control of infection and inflammation, moist wound management, and support for epithelialization, including care of the wound edges. A challenge in hard-to-heal wounds is bacterial biofilm, which is thought to be present in 60–80% of chronic ulcers vs. 6% of acute wounds. Therefore, it is crucial to pay attention to activities related to the eradication of biofilm from the wound and hygiene of the surrounding skin. In the presented case report, the acute wound was complicated. The use of the TIME strategy, the 4-step strategy of Wound Hygiene, and patient education supported the therapeutic process and enabled full healing after 6 weeks of therapy at home. key words: acute complicated wound, TIME strategy, home care, octenidine, wound hygiene |

Silver sulfathiazole (Argosulfan cream) is one of the most commonly used topical drugs for skin burn treatment. The dual mechanism of action of Argosulfan, based on antibacterial activity caused by silver and sulfonamide chemotherapeutic, provides efficacy in the treatment of wounds at risk of infection and ones that are already infected. This case report shows the efficacy of Argosulfan combined with absorbent dressings in the treatment of IIa and IIb skin burns. Pyoderma gangrenosum (PG) is a rare, potentially fatal skin disease with the formation of a painful, rapidly progressing (dynamic) necrotic ulcer with undermined blue-violet margins. PG in 50-70% of patients coexists with other systemic diseases: inflammatory bowel diseases (most frequent), arthritis (most often rheumatoid arthritis – RA), cancer of internal organs, myeloproliferative syndromes, and other dermatoses. 21-chromosome trisomy (Down’s syndrome – DS) is the most common autosomal genomic mutation among liveborn infants. Due to the coexistence of congenital defects, greater susceptibility to infections, autoimmune diseases, and greater risk of developing neoplastic diseases, patients suffering from DS require multidisciplinary medical supervision. The aim of the study was to present a 22-year-old patient with DS and coexisting PG hospitalized at the Clinical Department of Dermatology, University Hospital in Krakow. Pyoderma gangrenosum (PG) is a rare inflammatory disorder that may be a great masquerader in chronic wound diagnosis and treatment. PG is frequently misdiagnosed as venous insufficiency, arteriosclerosis, bacterial or fungal infection, or even vasculitis. The most common clinical variant is its ulcerative form, mainly found on lower legs. In many cases, PG may coexist with other inflammatory disorders like ulcerative colitis, Crohn’s disease, or rheumatoid arthritis. The aetiopathogenesis is unclear, but the immunological system plays a key role. We present a 59-year-old patient treated for several years for a venous ulcer that was finally diagnosed with pyoderma gangrenosum with characteristic pathergy phenomenon. The diagnosis was established based on clinical appearance and histopathological findings. The treatment of postoperative wounds after major surgery in obese and overburdened patients is a major challenge for medical personnel. This case report describes the process of complicated healing of a surgical wound after surgical removal of recurrent vulvar cancer. Due to the obesity of the patient and the stretching of the fatty tissue on the abdomen, the postoperative wound de-split, which resulted in bacterial colonization of the wound and hindered healing. Huge obesity made nursing activities more difficult. Difficult healing may also be related to the fact that the patient was burdened with diabetes and arterial hypertension. key words: vulvar cancer, wound hygiene, wound healing |

Introduction The nutritional status of a patient has a direct impact on the course and outcome of treatment. Early identification of malnourished patients allows for rapid nutritional intervention, which may improve their overall condition and help to optimise planned therapy. Nurses, whose knowledge and experience in this area can be crucial in the diagnostic and therapeutic process, play a special role here. The aim of the study was to assess nurses’ knowledge and competence in nutritional counselling and to find out their opinions on the importance of nutrition in chronic wound management. Material and methods The study included 70 nurses from surgical wards and primary health care. A questionnaire survey was used. Assessment of the respondents’ competence in nutrition education was measured using a 5-level Likert scale. Results The nurses’ level of knowledge was satisfactory. The statistical analysis showed that nurses who received training in wound care during their postgraduate education rated their competence in the area of nutritional education of a patient with a chronic wound better (χ2 = 5.927845, p = 0.0149). It was also shown that primary care nurses were more likely to pay attention to the quantity and quality of the patient’s food intake (χ2 = 5.876543, p = 0.0153). 68.57% of respondents stated that the diet of a patient with a chronic wound has a definite influence on the course of treatment. Conclusions Issues concerning the nutrition of patients with chronic wounds should be an indispensable part of the education of nurses at all stages of pre- and postgraduate education. key words: nutrition, chronic wound, nurse, level of knowledge |

The initial stage of wound healing is wound bed preparation by removing devitalised necrotic tissues. Debridement should be performed as quickly as possible to shorten the inflammatory phase and create optimal conditions for tissue regeneration. The implementation of the most favourable method of debridement should be thought out, potentially safe for the patient, and achieved in the shortest possible time. Wound debridement with surgical instruments combined with subsequent biodebridement is a safe and convenient method performed in patients under home care. The objective of the study was to review the literature on larvae therapy and wound debridement in patients under home care as well as related legal acts. A review of scientific papers and existing legislation was conducted based on the PubMed, Cochrane Library, and Termedia databases. Legal regulations and the competence of nurses create opportunities for specialised home care. Debridement with surgical instruments reduces the time of wound bed preparation and bacterial burden, thus accelerating regenerative processes in a wound. Advanced nursing care can improve the functioning of the health care system by greater involvement of nursing personnel in wound treatment and prophylaxis against wounds, which will result in decreased costs of hospital treatment. key words: Lucilia sericata larvae, wound debridement, ambulatory specialist care, hard-to-heal wound |

Over the last decade the medical larvae of Lucilia sericata have been claimed to be “miracle therapeutic maggots” due to their manifold biochemical properties that stimulate healing processes in a wound. Isolating chemical substances from maggot excretions and secretions gives greater possibilities to develop research into the use of defensins to stimulate healing processes in wounds of different aetiologies. Current studies focus on isolation, identification, recombination, and production of selected substances. A critical analysis of literature was done, based on studies published in the years 2000–2022. The authors conducted a review of scientific papers using the PubMed and Termedia databases using the following keywords: defensins, Lucilia sericata, maggot debridement therapy. Lucilia sericata excretions/secretions have proven antibacterial, antibiofilm, anti-inflammatory, and synergistic properties with specific antibiotics effectiveness. Efforts should be made to increase the level of knowledge and engagement among clinicians to implement and recommend MDT not only for biodebridement, but also to stimulate regenerative processes in a wound. keywords: biofilm, defensins, Lucilia sericata, chronic wound |

The paper deals with the timeless problem of the occurrence of pressure ulcers in patients. The paper focuses on possible ways to reduce the risk of developing this type of wound. The discussed issues are presented in the literature as significant medical problems that worsen the patient’s overall condition. The risk of developing pressure ulcers is determined by the number of risk factors present in the patient and the quality of care provided, among other factors. The aim of the study is to emphasize the importance of decubitus ulcer prevention based on the current literature and the patient’s description with multiple risk factors. The paper points to preventive actions and their positive influence on the patient’s condition.
