On June 15-17 this year, Kazimierz Dolny took place Scientific and Training Conference of the Polish Wound Management Association under the motto “Together we create the history of wound treatment in Poland”. There are numerous lecture sessions and workshops for this time, all in terms of daily practice. Specialists practising various medical professions met to discuss the treatment of hard-to-heal wounds.
Infrared thermography (IRT) is a non-invasive method that enables remote and non-contact assessment of temperature distribution on the surface of the body. In the prevention and treatment of chronic wounds, it allows accurate tracking of the thermoregulatory process in the limb or the whole body along with mapping the body surface temperature.
Leśniowski-Crohn’s disease is a risk factor for surgical site infection. We present a case of a 62-year-old patient with complicated Leśniowski-Crohn’s disease. Due to the acute phase of the disease, local advancement of the inflammatory process, the presence of peristomal fistulas and abscesses, as well as parastomal hernia, the patient developed a deep surgical site infection, requiring three re-operations. In the case of numerous, large wounds covering a large area of the abdominal wall with the presence of a stoma, the use of negative-pressure therapy with instillation allows for the optimization of the wound treatment and healing process. Wounds that cannot be treated with conventional wound care procedures or those that do not become sufficiently smaller with 6 weeks of treatment are usually considered for reconstructive procedures. The present study reviews surgical procedures based on the reconstructive ladder used for the treatment of difficult wounds that contain cavities or are located on vital organs or tissues and do not heal with standard wound care. This review, which highlights that using conventional techniques in such wounds may be a waste of time and money, is intended to guide health professionals dealing with difficult wounds.
It was noted that the presence of anaerobes is associated with deeper and more severe infections. However, due to the small number of literature reports, anaerobic infections are often not considered in therapy.
